We have played this dream all the way through, have searched every tile, and read every script and card. We ended up having to lose , because nowhere in all of this have we found the instruction to reveal card A, which is necessary to complete the dream. Can anyone tell me where this instruction is? It must be near the beginning, around the time the cellar is revealed, but we just can’t find it mentioned.

You're not actually likely to find it until late in the dream. Check the bottom right of C4. (Tough dream (maybe easier with multiple dreamers) I only succeeded on my second attempt after exploring most of it the first time.)
